Comments:
An unannounced monitoring inspection was conducted on 5/24/2023. The inspector was on site at the center from approximately 09:49am to 1:45pm. There were 47 children in attendance and a total of 10 staff present. Interviews were held with staff throughout the inspection, and the
inspector interacted with children in each classroom when appropriate. The center?s playground, medication and all classrooms were inspected today. Five children?s records and five employee records were reviewed during this inspection. There were violations found during today?s inspection. See the violation notice for more details.

Violations:
Standard #: 8VAC20-780-70
Description: Based on record review, the center did not ensure one of five staff records contained all of the required documentation.
Evidence:
1. The record of staff #5 (employment date: 7/1/2019) did not have documentation of the following: two or more references, orientation training and child abuse and neglect training.

Plan of Correction: Staff #2 is a seasonal employee who was hired in the summer of 2019 and has not worked at the center since the summer of 2021. Staff #2 just returned in May 2023. The Director was in the process of performing a file audit on Staff #2?s file. The Director has updated all missing paperwork with Staff #2. Going forward, the Director will make sure all paperwork is updated before any seasonal staff members return.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not
ensure areas and equipment of the center,
inside and outside, were maintained in a clean,
safe and operable condition.
Evidence:
1. The outside fence near the blacktop on the
playground is leaning, presenting a falling
hazard.

Plan of Correction: The fence construction has been contracted out and approved in March 2023. The company is waiting on materials and are hoping to start on replacing the fence in mid/late June 2023.

Standard #: 8VAC20-780-330-B
Description: The center did not ensure that playground
equipment had resilient surfacing that complies
with the minimum safety standards when
tested in accordance with the procedures
described in the American Society for Testing
and Materials.
Evidence:
The preschool slides, toddler slides and the
ground underneath and around the swing set
seats had approximately 0 - 5 inches of a resilient
material. The equipment required a minimum
of 6 inches of resilient material underneath and
around the swing set seats, slides and in the
fall zones.

Plan of Correction: The mulch will be replenished after the construction of the fence and sidewalk is complete. They are hoping to start in mid/late June 2023.

Standard #: 8VAC20-780-550-P
Description: Based on record review, the center did not ensure to maintain written injury records with all required documentation.
Evidence:
1. Four of five injury records reviewed did not contain documentation of the date and time when the parents were notified.
2. Three of five injury records did not contain documentation of how the parents were notified.

Plan of Correction: The parents were notified via email by administration. Going forward, administrators will review all incident reports and make sure all areas of the incident reports are filled out completely.
